Open Enrollment: 
To request changes, hard copy change forms, properly completed with original signature, must be received in the Insurance Office no later than October 31, 2025 by 5:00 p.m. and will be effective January 1, 2026. 

  • If no insurance changes are desired, no action is needed.
  • PPO/EPO: New Third Party Administrator: Anthem Blue Cross
    • Network remains Anthem Prudent Buyer PPO
    • Expect new cards for use as of January 1, 2026.
    • Member ID cards are not initially sent for dependent children; cards are available digitally or by mail through the Sydney App

  • PPO subscribers only: When a non-contracted provider has been paid for services, a super bill (detailed, paid receipt) should be submitted to the GGUSD Insurance Office.  Allow up to 8 weeks to receive an Explanation of Benefits  including a check for any reimbursement due you.
