Open Enrollment:
To request changes, hard copy change forms, properly completed with original signature, must be received in the Insurance Office no later than October 31, 2025 by 5:00 p.m. and will be effective January 1, 2026.
- If no insurance changes are desired, no action is needed.
- PPO/EPO: New Third Party Administrator: Anthem Blue Cross
- Network remains Anthem Prudent Buyer PPO
- Expect new cards for use as of January 1, 2026.
- Member ID cards are not initially sent for dependent children; cards are available digitally or by mail through the Sydney App.
- HMO:
- Providence Medical Groups now available. For a new PCP/Medical Group assignment, call 833-913-2236; Allow up to 6 weeks to change.
- Effective 1/1/26: (HMO only) Limited IVF services are covered as well as underlying services, diagnostic and related procedures.

PPO subscribers only: When a non-contracted provider has been paid for services, a super bill (detailed, paid receipt) should be submitted to the GGUSD Insurance Office. Allow up to 8 weeks to receive an Explanation of Benefits including a check for any reimbursement due you.
